---I haven't seen anyone offer the circuit boards yet. I know there was a
company on US-31 in Manistee, Michigan (was down the road from me) that
would build a board for anything as long as you had one they could work with
or had a print.

---The bulbs twist out (to the left) like they do in any vehicle. The holder
is a 5/8" socket that can be found at local auto parts stores like NAPA.
Sometimes a retaining clip breaks off the body of the socket and prevents
the connection from being completed, or, light sanding to the terminals and
the board with 1500 grit wet-sand to clean them works. The bulbs are #158
and they are not as easily found, but IIRC, Advance carries them, and
Discount Auto Parts. Wal-Mart claims they will order them if you need them,
but I haven't received that call and it has been nearly 3 years.

---FWIW, I have a bunch of rectangular IH two-prong flasher relays. I think
I might wire one into my "Front Axle Engaged" light for added effects. LOL
