E28 M5 (E34 M5) Squishy brakes

I will place my money on number 1.

It is real easy (especially when using one of those one-man bleeders) to
let the fluid level in the reservoir get too low and re-introduce air
into the system. I have sucked the reservoir empty with the ez-bleeder
more than once so I know it can be done... It is also possible that you
did not completely re-tighten one of the bleed valves which could also
allow air back into the system.

I would go back and re-bleed the system one more time, looking for leaks
at each of the bleed valves as you go, and make sure that you keep the
reservoir topped up at all times.
