Sure, but there can't be that much price difference (either labour or parts)
between replacing a mechanical tensioner every belt change and rekitting a
hydraulic tensioner every belt change.

Agreed. Do I want something that might drop a little bit of oil around
(never seen any on the belt yet, and I've rekitted two very grotty
tensioners), or something that means I have to think about what gear I'm
parked in or might simply let go for no readily-apparent reason (e.g. Doug
Sedon's recent failure).

I'm sure Highwood (UK) or OKParts (Germany) would stock them. You should be
able to source from the Australian suppliers, but that's a long (and
expensive) way to get a part from Italy to the USA.

Sounds like an AlfaBill opportunity to me ...
