    There are four bolts that hold the axle assembly to the axle tube.  If I
remember correctly, if you take those two screws out of the brake rotor and
remove that, you can access those four bolts easily.
    Disconnect the parking brake cables.
    The bearing is a press fit on the axle and comes out of the housing with
it.  You will need a press to remove and replace the bearing.
    The trick is pulling the axle and bearing out of the axle tube.  Usually
they are stuck in there pretty tight.  Maybe putting the wheel back on and
using it to pull will work but usually I end up renting a slide hammer
puller.

I have stared at my axle and searched the digest archives without revelation.
What has to be undone to let the drivers side axle pull out of the tube.
Obviously the caliper has to get out of the way. Beyond that, well, I just
can't tell.
Sure there are the two screws on the outside of the rotor, but they can't have
that much responsibility.  Is it the four nuts on the backside of the rotor?
I do recall someone saying to put the wheel back on to assist in pulling out
the shaft, just dunno what's keeping it there.
