"Bang" when shifting 4th to 5th at high rpm

Sounds like the driveshaft is hitting the transmission tunnel. You need to
check gearbox mounts, especially the large rear one, as well as the engine
mounts although this latter problem shows up in other ways first. The
driveshaft centre support is also a suspect.
