Sounds like warped rotors.  Is there also pulsing under your foot?  Given
the wheel shaking, it's probably the fronts (which do most of the work
anyway).  Remember of you use your brakes a lot (not always track
driving...can just be a lot of stop and go on those Seattle hills). try to
let them cool off a little before you park and park the car in gear if you
can without the handbrake to avoid hotspots.  No, don't leave your car
brake-less on a hill, just on flats where reverse will hold fine.

"Not every time" I would suspect is from uneven pedal pressure and the fact
that they're not that warped.  You're not always quite clamping onto them
hard enough to force the warpage through the system back to your hands.

You may be able to get away with turning them, though new rotors are pretty
cheap ($100 a set).
