Have you checked the throttle position switch is closing on
full throttle and has clean wiring connections? A very badly
adjusted throttle cable may not even be pulling the throttle
plate far enough to close the contact.

Lack of fuel could be due to the fuel pressure regulator,
you'll need a fuel pressure gauge to check properly. But one
easy thing to check is the thin rubber hose which comes out
the top of the FPR and plugs into the underside of the hose
between the valve cover and the air intake (PCV hose). If
this vacuum line is badly plugged in, clogged or perished it
could cause a lack of fuel pressure under high vacuum
conditions.

I've also read of bad DME connections causing this. I'm sure
there's is a FAQ on this (common?) wiring problem somewhere?

Run some Techron through the fuel tank in case it's injector
dirt build up (unlikely). How old is your fuel filter?
