The shaft does not have to twist. The amount of air passing through the
carb at idle is tiny, so only a very small difference is required to be
noticeable. Such a difference can easily be generated from leaks on
the seals on the throttle shaft, wear in the car bodies, and probably
most noticeable of all valve clearances changing with time.
