Darren,
Does it go away when the engine gets warm? Did you check the rest of the exhaust manifold nuts? Or the manifold to downpipe nuts? If it makes the noise just before it catches I tend to think that the noise is exhaust related. It also could be that gasket on the port you found the nut MIA on may be bad. This could be why it went away for a day you put the new nut on but is back now.
