They have direct fit and universal versions for many cars. 

I just looked and the direct fit cat for the 91 Alfa Spider has a P/N of 
AR1204 and lists for about $385.

The universals are listed by inlet, outlet, and length and range from $60 to 
$85.

I put one on one of my (non Alfa) cars and it worked great.
