Q: the US manifold has the 2 outlets side-by-side, whereas it *appears*
the only way the Euro manifold will go together properly is with the
outlets front-to-back.  This correct, or am I simply holding the parts
wrong?

I can't quite follow this description. The outlets on the Euro manifold go on
each side of
the motor mount, whereas the outlets to the US one are both to the rear of the
motor mount. Is
this what you mean? 

Q: the 2-piece manifold has threaded holes that look like air-injection
points, and all the photos I see of these things assembled shows them
filled with bolts.  Will the US air pump plenum bolt to the Euro manifold?

A: No, you have to make a special plug and drill and retap. In addition, #1
cylinder hole is too
close to #2. You have to cut and weld the air rail to bring this one closer.
Also, the new 
angle will not allow the pipe to go around the back of the head. Better to
start with an air rail
from a spider that goes around the front and then modify the #1 pipe. I had
Merrit Carden in
Fremont make the special plugs and Dennis over
at Norman Racing modify the air rail. All that 
custom work costed a pretty penny.
