I can address a couple of the questions based on
my recent experience with a garage addition:
I didn't do any anchor installations. One thing to
check is the concrete pressure rating - you want
4000PSI, if memory serves, for later lift installation.
Also you want at least 4" thickness - 5" is better,
or put a thicker pad under the area where the lift
posts will go.
Also, you should use some reinforcing rod/wire,
and a good vapor barrier under the slab will cut down
the moisture coming up through the concrete.
There are so many concrete paints and sealers out there,
that I gave up and just used plain old UGL sealer. That at
least stops a lot of the dusting of uncoated concrete. The
good paints/coatings do recommend etching. Cure waits
are on the order of 3 months or so for many products.
With 9'2" you can fit e.g. a 5000 lb Autolifters lift
(www.autolifters.com). But you will have only 5'5" or so
under the car. Drilling the 12 3/4" holes for installation
of expansion bolts in the floor is a pain without a proper
hammer drill.
