Milano gearshift rattle

It's doing it again :

The gearshift attachment at the linkage rattles such that I have to keep my
hand on the shifter to stop it.  My mechanic fixed it once about 50K miles ago
but it's time again.   

Any tips on doing it ?  What parts and tools might I need and will I need to
approach it from under the car ?  

Any long term affects if I don't fix it for a while ?
