Milano Power Steering

My '87 Milano Verde's PS is making some noise when I start it up cold.  It
sounds like it has air in the system.  I can hear the fluid in the system and
it changes when I turn the steering wheel.  Also when i'm parking it sometimes
there seems to be kind of a hitch when i'm turning the wheel.  Anyone have any
ideas?  The steering rack was replaced 3 years and 15k miles ago.
