SPICA cold starting procedures

I'm have a difficult time starting my '73 SPICA'd GTV when it is
cold; between 30-10F. My owners manual just sez to hold down the
accelerator pedal when cranking but that doesn't seem to help.
I pull out the throttle lever all the way out and hold the pedal
down about 1/2 way and then release. It never catches on the first time
the starter is engaged, but might on the second or third time; and when it
does it'll only run for a few seconds. Next try it will run a bit longer
and then finally it will continue to run but at very low rpms. If left
running, soon the rpms will pick up and then I have to push the throttle
lever in to bring the rpms back down. After pushing in the throttle lever,
it does fine.
Fuel pressure light goes out, has a rebuilt TA, and the SPICA has
been set up by a local Alfa mech. Starts and runs great in warmer weather.
What am I doing wrong?
