Shock Problem 87 325ic

I mounted monroe rear shocks with new upper shock mounts and have 
experienced mount failure.  The rubber within the mount separates, the shock 
piston retracts and falls loose.  Could someone advise me concerning the 
hardware (washers, etc) required for proper installation?  I don't have a 
parts explosion picture for this.
