Rocker Arm Clip - E34 525i M20

Haven't anyone experienced a broken Rocker Arm Clip before?. I recently
noticed that my engine was
extremely noisy and the tappets were even noisier. Upon opening of the top
cylinder cover, it revealed that
the rocker arm clip had broken off. I was lucky and spotted it early
otherwise the valve could have easily come
loose and gone right through the piston head!.

Is this a one in a million chance of a rocker arm clip breaking or is it a
known weakness?. TIA.
