Lock two nuts together on the stud, with enough thread clearance that you can screw it tight without the locking nuts tightening against the manifold.  Then just insert it, manifold in place, as you would any ordinary bolt.  On my Euro Alfetta manifold, I can simply finger tighten it into place.  My GTV has aftermarket headers, but I can't imagine it being impossibly different with a stock manifold.  (My apologies if it is impossibly different.)
