We know the Milano/75 is sensitive to prop shaft balancing.

Does it rev smoothly with the clutch pedal pressed?

If so the problem is rearward of the prop shaft.

Your description does sound like a clutch plate problem. Sounds exactly like
when the clutch on my (whisper it) BMW 535 gets hot.
