You have a couple of options. Do you have a mechanic's stethescope, the kind with the metal rod probe? (you might be able to improvise using a long screwdriver with the handle stuck in yer ear.)

Anyway, it's easy to hear the injector fire. It makes a *pink* noise. You could also go to the autoparts store and buy a injector test lamp. It's a little clear plastic button the size of a quarter, with a small bulb in it. You plug it into the harness, and the lightbulb lights when it gets the signal.

Oh I think you can also use a multimeter to measure the resistance of the injector. The service manual will tell you what it should be... but if it's zero or infinite that's probably not good.

Last year my Milano exhibited similar problems. I went through all the steps you've gone through, only to find that the injector screen was clogged. I put in a new injector and all was good.

If you get to the point where you are pulling the injectors, you might want to have them serviced. The electro-mechanical bits almost never go bad. I had mine cleaned and blueprinted (you'll get new seals, pintle caps, and filter baskets too) by http://www.cruzinperformance.com/ $11 per injector.
