164 Clutch slipping?

Last week whilst on holiday I began to notice that if I was driving at say 
40mph and I fully depressed the accelerator the engine revs would rise 
quickly but the car speed would take a few seconds longer.  My first thought 
is the clutch is slipping, but if I try a standing start only changing gears 
at 6000 rpm I cannot notice any slipping from the clutch.  Even when pulling 
away gently there is no slipping.  Could it be that during hard acceleration 
I am not noticing the slipping?

Any ideas, as this seems very strange to me
