Have you had a chance to check if the O2 sensor is working?  It may not be anymore...

No question, your catalyst is pretty much gone.  As Charlie mentioned, the HC and CO numbers are pretty marginal.  Usually, if all three are really bad, the catalyst is not working.  

But, the first thing I would check is the O2 sensor- the mixture might be reasonably close to where its supposed to be, but if the O2 sensor is not switching between rich and lean, then the catalyst will have a tough time working as intended.  Just connect a digital volt meter to it and watch with a reasonable load on it- it should switch around .5 volts (.2 to .8 if perfect, should work between .3 and .7, though).
