The blower speed control switch has four "out"
and one "in" wires. The high speed "out" wire
goes directly to the blower motor. The other
three "out" wires go to a blower resistor pack
and then one wire goes from the resistor
pack to the blower motor.
You have either a defective blower speed
control switch, a defective resistor pack, or
defective wiring. Locate the motor and resistor
pack and use a volt meter or ohm meter to
identify the problem.

Any obvious answers to why the heater fan only works on the high
speed setting?
