You can get some of the leaves out by going through the little servo 
opened vent/door from the passenger side.   Reach in a grab 'em out or 
use a vacuum.   Getting into the ducting above the fan is a bit more 
difficult... But if you are attacking the center console anyway go for 
it...  If you remove the fan you'll be able to suck out the rest.

Hard starting when cold:  Having had to replace one, I'd suspect the 
ThermoTime Switch.  http://www.alfaclub.org/techstff/bosch.htm has some 
decent trouble shooting info.

But to quote Mr. di Matteo:  "working witout a workshop manual is 
foolhardy.  Stop guessing and get yourself a manual or take your car to 
a qualified Alfa mechanic."

That said the Milano shop manual has an excellent bosch troubleshooting 
section (if you already have one, then look there... First chapter or 
two covers it in detail.)
