Our 1994 164LS blew out a plug last week. Turns out the head is striped. 

Since the plugs have been changed maybe 1-3 times over the course of 140k
miles, it's not like the threads were abused by insertion and removal.

Is this a rare event? Any other experiences like this? Your solutions?
